The short version: this is the website of a pilot product. You can read it, and you can email us. There is nothing to sign up for and nothing to buy here today.

What these terms cover

This page covers the UnityForm website and the demo conversations it invites. There are no accounts to create and no payments to take. If UnityForm starts charging money, real commercial terms — subscription, refund, and cancellation included — will replace this page before a single invoice goes out.

Pilot software, honestly labeled

UnityForm is not HIPAA compliant or certified today. The current prototype runs on synthetic data only and must not be used with real patient information. If you take a demo, keep real patient details out of it. The system watermarks every generated document as synthetic demo output for exactly this reason.

What UnityForm is not

UnityForm is administrative intake software. It does not diagnose, screen, triage, recommend treatment, or replace a clinician, qualified interpreter, or qualified translator. Nothing on this site is medical, legal, or compliance advice.

Using this site

The content, design, and screenshots here belong to UnityForm. The screenshots show the real product filled with synthetic demo data — if you share them, say that. Please do not present anything here as a claim we did not make; the pilot-status wording above is deliberate.

No warranties

The site and any demo builds are provided as-is and as-available, without warranties of any kind, express or implied, to the fullest extent the law allows. This is a pre-release product: treat everything here as evaluation material, not something to run a clinic on. To the same extent, we are not liable for damages arising from use of this site or of demo software. Some places do not allow certain of these limitations, so parts may not apply to you.

What is deliberately missing

There is no governing-law clause, arbitration section, or corporate entity named here yet, because UnityForm has not launched commercially. Proper commercial terms, reviewed by counsel, come before any paid launch. Until then, these are the honest ground rules for a website and a demo.
